Where do I send my new patient paperwork or my updated medical history paperwork?

We’ve made it easy to fill out all your new patient paperwork and your medical history update paperwork by making it electronically fillable. Please fill out the paperwork in its entirety and email it to us.

Do you see children and teenagers?

We do! We proudly see patients of all ages. From children all the way up to adults.

Do you offer braces?

We offer clear teeth aligners and they are amazing! No wires here.
